All plats submitted for approval of either minor site plan or subdivision, preliminary/major site plan or subdivision, final major site plan or subdivision, "a" appeal, "c" variance, "d" variance, "conditional use" and planning variance, shall include the information marked by an "x" in the Schedules below.
